Since 1978 there is a therapeutic possibility of helping childless couples, by using in vitro fertilization and embryo transfer, even when the Fallopian tubes are blocked.
If the man causes the main reason for infertility, there is a
very good chance of success, when the egg is fertilized outside of the body under certain conditions (
see Intracytoplasmic Sperm Injection). Basically one understands In Vitro Fertilization as
fertilization outside of the female body.The oocytes (egg cells) are taken out of the ovaries and are being merged with the sperm in a culture dish (=in vitro). 3 days later, the embryo resulting of the unification is transferred into the uterus (= embryo transfer).
Infertility is a very intimate problem for both partners. Therefore the treatment should be very
personal, and it should be carried out in a confidential atmosphere. The partners should be informed about all details and every aspect of the therapy associated with this method. After viewing all test results and having a complete gynaecological and semen examination done, a therapy plan will be issued. In many cases,
psychological consultation during the therapy is known to be pleasant for some patients.
After the preliminary discussion, every couple is given a therapy plan describing all medication. The treatment is planned individually, and, if necessary, is adapted to each particular situation. For the successful implication of in vitro fertilization, it is an advantage to have a few egg cells taken out. The greatest chance of becoming pregnant is being realized by reimplanting 2 to 3 embryos (in women over 40yrs , repeated sub optimal embryo quality) into the uterus.
The
Embryo Transfer can only take place when the woman has taken her prescribed medication precisely ( hormone tablets, injections etc.) This medication makes the body produce more eggs in the ovaries. The exact ovulation time can be calculated prior to this. This is very important because the egg cells must be extracted in the shortest possible time before the natural ovulation. The growing follicle can be examined when having an ultrasound check-up at a certain time (Picture 3). With this information and the hormone analysis ,we can work out the best time for egg extraction. Every couple is given a leaflet and therapy plan with accurate information about each examination. (ultrasound examination, hormone definition, blood tests etc)
